Braces Cost in Ohio 2026: $3,000 to $7,000 with Unusually Uniform Pricing Across Metros
Ohio orthodontic costs sit at the US average. Metal braces average $3,000 to $7,000 statewide. Invisalign averages $3,500 to $7,700. Ohio is unusual among populous states in that its three major metros (Cleveland, Columbus, Cincinnati) carry broadly similar orthodontic prices. Variation within each metro (downtown vs affluent suburb vs working-class neighbourhood) tends to be larger than variation between the three metros. Affluent suburbs add a 5 to 10 percent premium. Rural Ohio, including the Appalachian southeast, sits at the lower end of the state range.

Ohio Medicaid orthodontic coverage
Ohio Medicaid covers comprehensive orthodontic treatment for children under 21 with handicapping malocclusion meeting the HLD index threshold of 26 or higher. Coverage is administered through the Ohio Department of Medicaid. Cleft palate, craniofacial syndromes, and severe traumatic injuries are automatically approved.
Ohio Healthy Start (the state's Medicaid managed care for low-income children) follows the same orthodontic policy. CareSource, Buckeye Health Plan, Molina Healthcare of Ohio, Paramount Advantage, and United Healthcare Community Plan are the major Medicaid managed care contractors with provider directories.
Adult Ohio Medicaid orthodontic coverage exists only for reconstructive cases. The dental schools (OSU and Case Western) are the most reliable Medicaid orthodontic providers in the state.
Ohio dental schools offering reduced-cost orthodontics
- Ohio State University College of Dentistry Orthodontic Clinic (Columbus) : comprehensive treatment $3,000 to $5,500. Active orthodontic residency, accepts Medicaid universally. dentistry.osu.edu/patient-care.
- Case Western Reserve University School of Dental Medicine (Cleveland) : comprehensive treatment $3,200 to $5,800. Active orthodontic residency. case.edu/dental/patient-care.
Both accept Medicaid. OSU has a larger patient panel and orthodontic residency cohort; Case Western typically has shorter wait times. For Cincinnati-area families, both schools require a 1.5-hour or longer drive.
Ohio FSA / HSA considerations
Ohio state income tax is graduated, ranging from 2.75 percent to 3.5 percent for the highest bracket as of 2026. Combined with federal income tax and FICA, an Ohio FSA contribution generates a discount of approximately 28 to 37 percent depending on bracket. A $3,400 FSA election effectively pays $1,000 to $1,250 of the orthodontic fee from after-tax cash savings.
Some Ohio municipalities (Columbus, Cleveland, Cincinnati) levy local income taxes of 1.5 to 2.5 percent for residents, which slightly increases the FSA discount for residents of those cities. See our FSA and HSA strategy page for the worked math.
