Virginia Medicaid: Smiles For Children
Virginia Medicaid covers comprehensive orthodontic treatment for children under 21 with handicapping malocclusion meeting the HLD index threshold of 28 or higher. Coverage is delivered through Virginia Department of Medical Assistance Services (DMAS) and the Smiles For Children dental contractor (currently DentaQuest). Cleft palate, craniofacial syndromes, and severe traumatic injuries are automatically approved.
Virginia FAMIS (Family Access to Medical Insurance Security) is the state's CHIP program for children whose families earn too much for Medicaid. FAMIS dental coverage follows the same orthodontic policy as Medicaid for the dental component.
Adult Virginia Medicaid orthodontic coverage exists only for reconstructive cases. The dental school (VCU School of Dentistry in Richmond) is the most reliable Medicaid orthodontic provider statewide.
Virginia dental school orthodontic clinic
- Virginia Commonwealth University School of Dentistry (Richmond) : comprehensive treatment $3,200 to $5,800. The only orthodontic residency clinic in Virginia. Accepts Smiles For Children and most commercial insurance. dentistry.vcu.edu/patients.
For Northern Virginia families, the drive to Richmond is roughly 2 hours each way. Across an 18- to 24-month treatment with monthly visits, this is a meaningful cumulative travel investment. Many NoVA families opt for community-practice treatment despite the higher cost, or use the FSA / HSA strategy to reduce net cash out.
NoVA savings strategies
For Northern Virginia patients facing $7,500 to $8,300 quotes, the practical savings come from three moves.
First, drive west or south within Virginia. A McLean patient who travels to a Loudoun County practice (Leesburg, Ashburn, Sterling) typically saves $700 to $1,200 versus a comparable McLean or Vienna practice. A drive to Stafford or Fredericksburg can save $1,000 to $1,500.
Second, FSA / HSA strategy. Virginia state income tax (5.75 percent for the highest brackets) plus federal plus FICA gives an FSA discount of approximately 32 to 42 percent depending on bracket. A $3,400 FSA election effectively pays $1,200 to $1,500 of the orthodontic fee from after-tax cash savings. Federal employees in NoVA have particularly good access to FSA programs through the Federal Flexible Spending Account Program (FSAFEDS).
Third, in-house payment plans. Most NoVA orthodontic practices offer 0 percent in-house payment plans across the treatment duration. Combine with FSA election timing to spread cash flow across the case. See our financing options page and our negotiating playbook.
